If Clicked Yes

• A new Free Form field opens: Reason (Mandatory)
• Move application to a new status called “DNA”
• Adds a banner in upper left corner of profile saying “Do Not Adopt”
• Adds person to a “Do Not Adopt” Report which includes First Name | Last Name | eMail | Phone | Reason | Administrator’s Name | Date Added To list

Includes:

Ability to change back from yes to no in case of mistakes. If changed, the DNA report changes as well.
